What to Bring To Your First Appointment

What to Bring To Your First Appointment?

Your first appointment is the time when you and your chosen Doctor get to know each other.

Please bring with you any documents and information that might help that process, including:

  • Your referral from your GP
  • Any results from blood tests
  • Your latest ultrasound scan results and images

Late Arrivals

Please give yourself plenty of time to arrive and park and make your way to Evolve Women’s Health.

We want to minimize the waiting time for all patients and ask that all patients try to be on time.

For first time consultations there will be some forms to complete, so please arrive ten minutes early.

If you are running late, please give us a call on (07) 4688 5533, otherwise we can not guarantee you will be able to see the doctor on the day of your booked consultation.

Cancellations and No Shows

Please provide at least 24 hours notice for cancellations.

For No Shows, it is our policy to charge the patient the full amount of the consultation, except in extenuating circumstances.

Payment of Accounts

Please note we do require patients to settle accounts at the time of their visit.

For your convenience, we offer EFTPOS and credit card facilities (including AMEX) and can process your claim directly with Medicare if your details are registered. We do not accept cheques and prefer not to take cash payments.

Find out more information about this service, and how you can register with Medicare.

Follow Up Appointments

If a follow-up appointment is needed, we will try to book this at the time of your consultation.
